چکیده

rant artery or cerebellopontine angle masses [2, 3] . The offending vessels are usually arterial structures, such as the anterior or posterior inferior cerebellar arteries [4] . We present the case of a patient with HFS secondary to compression by a giant vertebrobasilar dolichoectasia (VBD) and highlight the possible association of HFS with VBD and type 1 neurofibromatosis (NF-1). A 70-year-old man complained of a 3-year history of intermittent, involuntary muscle spasms involving the left lower and mid portions of his face. Approximately 18 months before presentation, he gradually developed mild speech difficulties that progressed to moderate dysarthria with nasality without diurnal variation or ocular symptoms. His past medical history was significant for NF-1 and renal cell carcinoma (postnephrectomy) and was negative for hypertension or hyperlipidemia. Physical examination revealed multiple cutaneous neurofibromas and café au lait spots, with axillary freckling, confirming the diagnosis of NF-1 ( fig. 1 D). Neurological examination revealed intermittent spasms of the midand lower-left facial region consistent with left HFS. Mild dysarthria with nasality were also noted. The rest of the examination was otherwise normal. Plantar reflexes were flexors. Hemifacial spasm (HFS), characterized by irregular, involuntary and recurrent tonic and clonic contractions of muscles innervated by the ipsilateral facial nerve [1] , is usually attributed to compression of the facial nerve at the root entry zone by an aberPublished online: March 18, 2009

Hemifacial spasm and recurrent stroke due to vertebrobasilar dolichoectasia coexisting with saccular aneurysm of the basilar artery: a case report.

We describe a patient presented with sequential events of hemifacial spasm, cerebral infarction and fatal subarachnoid hemorrhage. All of them are seemingly separate entities. Radiological examination revealed that the cause was vertebrobasilar dolichoectasia (VBD) coexisting with a giant saccular aneurysm of basilar artery. VBD is a dilatation and elongation of vertebral artery (VA) and basila...

Secondary or Symptomatic Hemifacial Spasm caused by a Tumor of Cerebellopontine Angle Case Report and Review of the Literature

Background & Importance: Primary hemifacial spasm is usually caused by microvascular compression of the facial nerve at its root exit zone at brainstem without any space-occupying pathology. Secondary or so-called symptomatic hemifacial spasm has an additional underlying pathology e.g. tumors leading to the microvascular compression. We review and discuss the pathophysiology and the literature ...

The Effect of Microvascular Decompression for Hemifacial Spasm Caused by Vertebrobasilar Dolichoectasia

OBJECTIVE Hemifacial spasm (HFS) caused by vertebrobasilar dolichoectasia (VBD) is very rare, and in theses cases, it is difficult to decompress the nerve from its vascular compression. The objective of this study was to investigate the outcome of microvascular decompression (MVD) for HFS caused by VBD. METHODS There were 10 patients of HFS caused by VBD at our hospital between September 1978...

E-mail: [email protected] Vertebrobasilar dolichoectasia (VBD) is an anatomic variant that consists of enlargement and dilatation, often associated with a tortuous and elongated vessel. The anomaly is probably due to a marked thinning of the internal elastica lamina and media, most likely as a consequenc...

Hemifacial spasm in a patient with basilar artery dolichoectasia caused by uncontrolled hypertension

A 47-year-old male presented with a 2-year history of hemifacial spasm. Magnetic resonance imaging performed showed his tortuous basilar artery with nerve compression, and the patient was treated conservatively with botulinum toxin injections with complete resolution of symptoms. This rare disease was caused by his long history of hypertension, which led to his major basilar artery dolichoectasia.
